Primary Care of Southwest Georgia (PCSG) and its interdisciplinary staff provide comprehensive patient-centered primary care services to entire families and individuals in six locations serving Early, Decatur, Thomas, and Brooks counties and surrounding communities.
PCSG facilitates its patients’ access to all levels of the health care system through a family practice model of care and is especially committed to providing services for the medically underserved, regardless of the ability to pay.
PCSG was established in 2005 and opened as an FQHC in Blakely, Georgia, in March 2006.  In August 2007, a second site was opened in Thomasville, Georgia. PCSG expanded to a third location in Bainbridge, Georgia, in August 2015 and a fourth location in Quitman April 2017.  In January 2018, PCSG opened its first school-based health center at Thomas County Middle School and a second school-based health center in Blakely, Georgia, at the Early County Elementary School.
PCSG is a non-profit corporation that is governed by a local board of directors made up of community members, 51% of whom are required to be active patients.
PCSG is a strong advocate of preventative education and public health activities.  Primary Care of Southwest Georgia collaborates with other community agencies and organizations, i.e., local hospitals, Spring Creek Health Cooperative, and Public Health to identify health-related needs and to develop resources to facilitate these needs.
PCSG’s team of providers delivers family medicine, adult medicine, women’s health, pediatrics, behavioral health, telehealth, laboratory, and pharmacy services. PCSG is accredited as a Patient Centered Medical Home by the Accreditation Association for Ambulatory Health Care (AAAHC), which engages in quality assessment and quality improvement to facilitate the effectiveness of its comprehensive primary care services on the overall health of its practice population.
Primary Care of Southwest Georgia’s interprofessional comprehensive primary care clinical settings provide learning opportunities for MCG students to experience and participate in the delivery of care to underserved communities and populations. As an MCG FQHC/CHC Educational Affiliate, they will provide a variety of student-required and elective educational options under the supervision of physician members of their staff who also serve as MCG Community Clinical Faculty members.
