We are a community health center.
And Jesus is our model.
Christ Community Health Services (CCHS), founded in 2007, opened its Olde Town medical clinic in the Boardman Widows Home in 2011 in downtown 桃色视频 as a Federally Qualified Health Center (FQHC). CCHS now鈥痮perates鈥痜our community practice sites, one of which includes a separate dental practice site鈥痵trategically鈥痩ocated鈥痠n urban 桃色视频 neighborhoods, providing local access to affordable care for patients who face medical, economic, and transportation hardships.鈥
This FQHC, which is a not-for-profit Christ-based comprehensive primary healthcare practice, provides adult and pediatric primary care, HIV/chronic disease management, chronic care management, dental, laboratory, physical therapy, integrated behavioral health services, prescription assistance, and specialty referrals to more than 7,300 underinsured and uninsured families from a 12-county region in the Central Savannah River Area of Georgia and South Carolina. These services are provided by an interprofessional care team of physicians (internal medicine, family medicine, pediatrics, and behavioral health), nurses, nurse practitioners, physician assistants, dentists, licensed clinical social workers, and others. CCHS is also a Level III National Committee for Quality Assurance (NCQA) Recognized Patient-Centered Medical Home (PCMH) based on the presence of team-based care and coordination, patient-centered access, care management, and ongoing quality improvement in its clinical practices.
This faith-based practice鈥檚 mission is to proclaim, 鈥淛esus as Lord and鈥痙emonstrate鈥疕is love by providing affordable, quality health care access to all, especially those underserved.鈥
Christ Community Health Services (CCHS) has a long history of Medical College of Georgia (MCG) student education participation using its clinical platforms to provide required clinical student experiences as an MCG Educational Affiliate. These learning opportunities are supervised by members of CCHS鈥檚 physician staff who also serve as MCG Community Clinical Faculty members and are further鈥痜acilitated鈥痓y others in the interprofessional team of healthcare providers.鈥
